Does Kaiser Cover Services? Here's What Medi-Cal and Commercial Members Need to Know

If you have Kaiser Permanente, whether through a commercial plan or Medi-Cal, you may have doula coverage that includes lactation support, birth preparation, postpartum care, and a whole lot more. Here's how it works and how to access it.

What is Doula Coverage?

Doulas provide support across the full perinatal period, pregnancy, labor and delivery, and up to one year postpartum. Under both Medi-Cal and most Kaiser commercial plans, covered doula services include:

  • Prenatal and postpartum visits

  • Support during labor, delivery, miscarriage, or abortion

  • Health navigation and evidence-based education

  • Birth plan development

  • Lactation support

  • Newborn and infant care guidance

  • Linkages to community resources

This is broad, holistic support, and lactation support is included in the scope. As an occupational therapist, lactation consultant, and postpartum doula, this allows me to offer a wide variety of care to clients.

How Many Visits are Covered?

For Medi-Cal members, the standard authorization includes:

  • One initial visit

  • Up to 8 additional visits (prenatal and/or postpartum, any combination)

  • Labor and delivery support

  • Up to 2 extended 3-hour postpartum visits

Up to 9 additional postpartum visits are available with a second written recommendation from your physician or licensed provider (OB, midwife, etc.). Already postpartum and didn't use a doula during pregnancy? You can still use all of your visits during the postpartum period — up to one year after your pregnancy ended.

For Kaiser commercial members, doula services are covered as a standard benefit on most plans. If you're on a self-funded employer plan, coverage depends on whether your employer included this benefit; we verify this directly in the portal before your first visit, so there are no surprises.

What isn't Covered

Doula coverage does not include occupational therapy, manual therapy (craniosacral therapy and myofascial release), or clinical lactation consulting. Those fall outside the doula's scope of practice. If those services are part of your care, we'll walk you through options and costs before anything is scheduled.
